This is a 48 year old chronically obese diabetic who has lymphedema, severe venous insufficiency and Charcot arthropathy. Here you can see all the verrucous epidermal hyperplasia from his long-standing lymphedema.
He also has a large venous ulceration on his left lateral ankle that we have treated but it has recurred because of him being non-compliant and not wearing his pressure off loading boots while he is sleeping. He had this severe onychomycosis, onychocryptosis and his nailed was avulsed.
Given the fact he’s on Plavix and aspirin, in the office we started with Lumicain for three or four minutes to get it to stop bleeding, but it wasn’t. This required the use of the OMNI-STAT to control the bleeding and then we applied advanced topicals over that with his multi-layer compression bandaging.
